Appointment Information

I'm currently offering virtual appointments to patients in Alberta, Canada. 

In person appointments will be available in St. Alberta, Alberta beginning August 2026.

Initial Intake appointment (adult)

75 minutes

Your initial intake appointment is an opportunity to get a full picture of your health, lifestyle, and goals. During this session, we’ll review your medical history, health goals, current concerns, lab results, and any previous treatments. I'll provide treatment recommendations, education, and an opportunity to ask questions. 

After booking your initial appointment, you’ll receive a comprehensive New Patient form to complete before your visit. To help you prepare, I’ve outlined a few key areas below so you can gather any relevant information in advance.

Not everyone has access to their family history and that's okay, we'll work with what we have - do your best with what you know and can find out.

Helpful information to know about your family history:

  • Chronic conditions (hypertension, diabetes, autoimmune disease), cancer, pregnancy outcomes, genetic mutations, mental health

Medications & Supplements

Prescription and over the counter medications: name, dose, frequency, duration, reason
 

Supplements: brand, name, dose, frequency, duration, reason
 

Current and past responses/sensitivities to medications/supplements

Labs & Imaging Results

Recent bloodwork
 

Imaging results (MRI, Xray, DEXA, ultrasound, PAP etc)
 

Fertility specific testing (semen analysis, AMH, etc)

Tracking Information

Menstrual cycle tracking (Flo, Clue, other apps) & Ovulation tracking (BBT, LH strips, fertility monitors)

VIRTUAL FOLLOW UP (ADULT)

30 minutes

Follow-up appointments are a chance to check in, see how things are going, fine-tune your plan and provide ongoing support. We’ll review your progress, discuss lab results, symptom tracking, and make any adjustments needed to help you feel your best. 

VIRTUAL FOLLOW UP (ADULT)

45 minutes

This longer follow-up is ideal for patients who would benefit from additional time to explore their care more thoroughly. It’s well suited for those with complex or multiple concerns, detailed lab or functional testing to review, anyone looking for more in-depth education and discussion around their treatment plan, or those transferring from another care provider within the clinic.

This visit allows us to move at a more comfortable pace, answer questions fully, and make thoughtful, personalized adjustments to your plan so you feel clear, supported, and confident moving forward.
